Unfortunately 3 people were killed. The caravanners who were in their 80's and a car driver.
I can't figure out how you can get on a motorway in the u.k on the wrong side. They all have slipways...

That's very sad, that a wrong turn led to a lost of lives.
Regarding as to how a wrong turn was taken, it can happen quite easily when driving using a sat nav or Google maps. Driving in Ireland or uk, and take the third exit on a roundabout means that you are going right. However in Europe it means that you are going left.
A moment of tiredness or disorientation can cause a dangerous situation very quickly.

In THIS EXAMPLE a vehicle already on the incorrect side approaching along the B430 could easily continue on taking the turn right onto the exit ramp and end up travelling.on the wrong carriageway of the M40 against oncoming traffic.

Going around a roundabout at the entry and exit to a motorway, a foreign driver can follow his old habits driving on the left and turn down the exit road. How many on their first continental trip in a morning rushing to get under way suddenly realise , and say Tut Tut I dont belong on this side or words to that effect, How many times do you see a sign like scull and cross bones telling you not to enter a slip road, do you think our signage is too polite?
