anyone know if its possible to extend this bike rack so that it can hold 4 bikes. looks like there's hole for a third rail but i'm wondering if i loosen all the bolts does it pull out to take 4

I would guess that if need be you could extend the bottom bar to carry four. Whether the top clamp fittings would reach the outer bikes might be an issue, although there must be new carriers for four so extra arms should be available.
You may have to consider the extra weight this will exert on the wall fittings and what effect this has on the rear axle loading if you are like me already near the max.
I suspect there is also a maximum distance from the back of the van that the load protrudes before you need an extra lighting board.

Are the top and bottom fixings through structural members, or just through the outer skin? There'd be fair load on that with extra bikes.

Looking at your avatar would I be right in thinking that your van is a rimor brig 648 on twin wheels? If so you should have plenty payload for 4 bikes. I have seen lesser vans with 4 bikes on the back of them.

Yeah your spot on rimor brig 648 should be fine with the weight
I have a Fiamma 4 x bike rack and the max weight it will carry is 60kg (a lot of bikes weigh over 15kg). Despite it being checked during the annual habitation check, I have never carried four bikes on the back - don't like the idea of the weight/overhang on the back panel. I have seen a couple of vans with interesting crease lines

I suppose if he need to take four bikes and was concerned with the strength of the back wall he could put a piece of light pipe or box iron from the towbar up at an angle to the bottom of the rear bar of the Bike rack, that is if he has a tow bar or some suitable anchor point. That would eliminate the outward pull on the top brackets of the bike rack.

We had a 4 bike rack on a camper and two wheel supportscould be removed and the bottom cross tube slid into the bigger tube.
From memory, I think the holes for the retaining bolts matched up when tube was either in or out. This might indicate whether it is designed to be extended or not. I did put a timber (piece of architrave) on the inside of the wall with two light " L" brackets onto the wood and side walls. Over spec. maybe, but was concerned about the weight, especially after going over a bump.
I also had a light aluminium tube going down to the bumper (at about 45 degrees) from the outside tube.
I bent a bit of aluminium sheet into 2 Us to make a small bracket for the tube and put a pin through bracket and tube.
The brackets were pop riveted to the bumper and bike rack tube.
